Autologous Facial Fat Grafting

Autologous facial fat grafting restores the volumetric dimension of facial youth — the fullness of the midface, temples, and periorbital region that deflates with age. Using the patient's own fat, it is the most biocompatible and natural method of facial volumization available, producing results that integrate seamlessly with surrounding tissue and, for the surviving graft, remain permanently.

A facelift repositions and lifts descended tissue. Fat grafting restores what has been lost. These are complementary — not competing — interventions, and the most natural-looking facial rejuvenation results are achieved when both are addressed. As the facial fat compartments deflate over time, the face develops hollowness, shadowing, and a skeletonized appearance that no amount of lifting can correct. Dr. Lemelman harvests fat from a donor site — typically the abdomen or inner thigh — using a small-cannula technique that preserves fat cell viability. The fat is processed in a closed, sterile system and reintroduced through blunt micro-cannula injections in precise micro-aliquots at multiple tissue planes, following the principles of structural fat grafting (Coleman technique). Target areas include the midface fat compartments, temporal fossa, tear troughs, perioral region, and lips. A portion of the transferred fat establishes a permanent blood supply and remains indefinitely; the non-engrafted fraction resorbs over 3–4 months.

A brow lift elevates a descended or asymmetric brow to its youthful position, opening the upper face and restoring the natural arch that defines a rested, alert expression. It is one of the most effective procedures for correcting the heavy, stern appearance caused by brow ptosis.

Brow descent is a primary driver of upper facial aging — and one of the most commonly overlooked. As the brow falls below the orbital rim, it compresses the upper eyelid and creates a hooded, fatigued appearance that upper blepharoplasty alone cannot fully correct. Dr. Lemelman performs the brow lift through small, concealed incisions behind the hairline, repositioning the brow and forehead tissues without a long scar. Sometimes a direct brow lift, using an incision directly above the lateral eyebrow is a good technique depending on the individual's anatomy.
